The step people forget

You finished your braces or aligners. Your teeth look great. It is tempting to think the work is over. It is not. The final stage of orthodontic treatment is retention, and skipping it is the most common reason teeth drift out of place afterwards.

Why do teeth move back?

Teeth sit in bone, held by fibres in the gums and surrounding tissue. After teeth are moved, those tissues are still adapting and tend to pull teeth back toward their old positions. Teeth also change slowly throughout life with ageing, chewing forces and habits like tongue thrusting or grinding. A retainer holds teeth steady while tissues settle, and keeps them there in the long term.

Types of retainer

Retainer types: removable clear, Hawley, and fixed bonded wire

  • Clear removable retainer. A thin, transparent tray, similar to an aligner. Discreet and easy to wear, but needs care to avoid loss or warping.
  • Hawley retainer. A wire and acrylic plate. Durable and adjustable.
  • Fixed (bonded) retainer. A thin wire bonded behind the front teeth. Works all day without you doing anything, but needs extra attention when cleaning.

Many people use a combination. Your orthodontist will advise what suits your case.

How long should you wear them?

Usually full-time at first, for a few months, then at night for the long term. Many orthodontists recommend wearing retainers at night indefinitely, or at least several nights a week, because teeth can keep moving throughout life. Your orthodontist will give you a schedule that fits your case.

Looking after your retainer

  • Keep it in its case whenever it is not in your mouth. Retainers wrapped in napkins end up in the bin.
  • Clean it daily with a soft brush and lukewarm water, or as advised. Avoid hot water, which can warp the plastic.
  • Keep it away from pets, who love chewing them.
  • Clean around fixed retainers with floss threaders or interdental brushes.
  • Bring it to your check-ups.

What if it breaks or no longer fits?

  • Broken or lost retainer. Call us promptly. A replacement is usually simple if you act early.
  • Retainer feels tight. Teeth may have moved slightly. Wear it as advised and contact us. Do not force it.
  • Fixed wire has come loose or poked out. Do not pull at it. Contact us for a quick repair.

If teeth have already moved a lot, a short course of aligners may be needed to realign them.

If your retainer is not in your mouth, it is not doing its job. Think of it as the permanent insurance for the time and effort you invested.
